Frozen Shoulder

Shoulder Pain Marpole Treatment
The subscapularis is often the "forgotten" muscle of the rotator cuff, yet it is the largest and strongest of the four. Located on the front of the shoulder blade, it provides the essential front-side stability needed for almost every upper-body movement.
At Granville Physio, we specialize in identifying and treating subscapularis dysfunction—especially when it has become a chronic source of pain that hasn't responded to traditional rest.
The Role of the SubscapularisThe subscapularis is your shoulder's primary internal rotator. You rely on it for:
  • Reaching behind your back (e.g., tucking in a shirt or fastening a bra).
  • Power movements like pushing, throwing, or swimming.
  • Joint Centration: It acts as the "front anchor" that prevents the shoulder bone from sliding too far forward in the socket.
When this tendon is strained or torn, you may feel a deep ache in the front of the shoulder, experience weakness when pushing, or feel "clunking" and instability during movement.

How Shockwave Therapy Restores Subscapularis FunctionBecause the subscapularis is tucked deep behind the ribs and pectorals, it can be difficult to reach with standard manual therapy. Extracorporeal Shockwave Therapy (ESWT) uses high-energy acoustic pulses to penetrate deep into the tissue, triggering a biological "reset."
1. Advanced Tissue Repair (Mechanotransduction)Shockwave therapy doesn't just mask pain; it physically repairs the tendon at a cellular level:
  • Neovascularization: It stimulates the growth of new blood vessels, flooding the tendon with the nutrients required to heal partial-thickness tears.
  • Collagen Synthesis: It promotes the production of Type I and Type III collagen, replacing weak scar tissue with strong, functional fibers.
  • Growth Factor Release: It activates the body’s internal repair proteins to accelerate regeneration.
2. Rapid Pain Relief
  • Nerve Desensitization: The acoustic waves provide "hyperstimulation anesthesia," temporarily quieting the overactive pain receptors in the front of the shoulder.
  • Substance P Reduction: It lowers the concentration of Substance P, the chemical messenger responsible for chronic, "nagging" pain.
3. Resolving Calcific TendinitisWhen calcium deposits form in the subscapularis, they can cause a physical block in your range of motion. Shockwave is the gold-standard treatment to fragment these deposits, allowing your body to reabsorb them naturally and avoid surgical intervention.

Clinical Evidence: Why We Choose ShockwaveResearch in the Journal of Orthopaedic Surgery and Research has highlighted that for subscapularis and general rotator cuff injuries:
  • ESWT significantly improves functional scores and reduces pain more effectively than physical therapy alone.
  • Long-term recovery rates are higher when Shockwave is used to treat chronic tendinopathy compared to corticosteroid injections, which only offer short-term relief.

Stop Struggling with Front-Shoulder PainAt Granville Physio, we combine the deep-reaching power of Shockwave Therapy with a targeted strengthening program to restore your shoulder’s front-side stability.
